Below is the framework for all terms and conditions, consent agreement and services at Horizon Diet and Nutrition Advisors Ltd.

Please talk with our Nutritionist team at info@horizondn.com if there is anything you would like clarified before you sign or click the consent form for our nutritionist to provide you with nutritional therapy.

After completion of the questions on the questionnaire web page, you will be requested to agree with the terms and conditions, provide consent and submit the questionnaire.

Once submitted our nutritionist will provide you online nutritional therapy via skype or zoom.

Horizon Diet & Nutrition Advisors does not provide nutritional therapy to anyone under the age of 18.

Client/Patient must be above the age of 18 to receive nutritional therapy.

Horizon Diet & Nutrition Advisors Ltd does not recommend or grant permission to any client to share their diet plan with any member of the public. Each diet plan is uniquely developed for the client only and not for the general public. 

Horizon Diet & Nutrition Advisors Ltd does not take any responsibility for any client sharing any nutritional therapy information provided by Horizon Diet & Nutrition advisors Ltd.

If any client decides to shares their own customised diet plan with others, then they have agreed to take full responsible for any damages caused to anyone in the public.

Horizon Diet & Nutrition Advisors Ltd does not recommend any client to share their diet plan with any member of the public.

Online Nutrition Therapy

Horizon Diet & Nutrition advisors ltd uses Fresha applications for all bookings and payments processes for online nutritional therapy.

Our services are available in the United Kingdom and other countries. 

All documents emailed will be in a PDF format and it is the responsibility of the client to have PDF reading software on all electrical devices.

Any client/patient who purchased any product from our website and uses another name in the questionnaire form will need to confirm the are correct identity and authorisation. 

Confirmation can be done on the questionnaire form or via email, if the information is not confirmed or inaccurate we will refund the payment into the account of which we have initially received payment.

Any fraudulent actions or abusive actions towards our staff will be reported to the Police.

It is the responsibility of the client to complete an online questionnaire form correctly and truthfully to avoid any complications. 

Our Nutritionist cannot be held accountable or liable if the information is not presented correctly by the client.

After payment is completed of Email-based diet plans, the client will be re-directed to a questionnaire form which needs to be completed fully by the client/patient within 14 days. 

Once the form is complete and consent has been approved by the client to provide nutrition therapy, Horizon Diet and Nutrition Advisors Ltd Nutritionist will respond to the client/patient request within 1 to 7 working days.

Service will be terminated or rejected by our Nutritionist if the client/patient perform any of the following actions:

Client/Patient using abusive/threatening language or racially discriminates towards any member of staff.
Client/Patient displaying inappropriate images of any sort to any member of staff.
Client/Patient discussing non-nutritional therapy-related matters with any member of staff.

Skype/Zoom/Team communication requirements:

The questionnaire form must be submitted before the appointment time, failing to do so will result in Horizon Diet and Nutrition Advisors Ltd nutritionist not being allowed to provide service.

For residence of USA and Canada, by accepting our terms and condition's you accept to pay all transatlantic costs of our insurance company Westminster insurance if you the resident of USA/Canada want to make a claim.

Skype/Zoom communication will not be able to continue if the questionnaire form is not completed.

The client is responsible to have the correct devices and internet speed for Skype/Zoom communication.

All Nutritionists will mainly communicate in English.

Online Nutrition Therapy refund policy.

Clients have the right to a refund within the limits set out in the terms and conditions.

We at Horizon Diet and Nutrition Advisors Ltd only provide refunds on the following:

Client/patient paid for service and doesn't receive diet plan documents within 7 working days will be refunded.

Skype/Zoom/Teams communication is compromised at Horizon Diet and Nutrition Advisors offices.  

If our nutritionist cannot provide a diet plan for a client/patient due to there major health condition. We will provide a full refund to the client/patient.

We will not transfer any refunds to another account.

The client will need to email nutrition@horizondn.com to request a refund.

When a refund request is reviewed and granted we will refund the clients account of the full-service price within 3 to 5 working days.

No refund will be issued for the following:

The client/patient purchases any service and doesn't agree with the information or instructions provided by our nutritionist.

Client/patient who misses Skype/Zoom/Team appointment without cancelling an appointment or has communication issues which are not resolved within the time frame of each appointment booked.

Client/Patient using abusive/threatening language or racially discriminate any member of staff.

Client/Patient displaying inappropriate images of any sort.

Client/Patient does not complete or submit a questionnaire form before or within the scheduled appointment time.

Payments methods

Appointment payments can be made within the Fresha application. 

If no notice is given to cancel any session/sessions you will be required to pay for the missed/cancellation fee. 

You must attend the sessions regularly. However, if you do need to cancel an appointment please let your nutritional therapist or our admin team know with at least 24 hours notice. You can do this by emailing info@horizondn.com or nutrition@horizondn.com

It is confidential that you are a client of Horizon Diet and Nutrition Advisors Ltd, as well as what you discuss in your sessions. There are, however, some limits and exceptions to confidentiality.

If our nutritionist has serious concerns about your safety or the safety of another person normal confidentiality may be lifted.

You will be asked to provide your GP’s contact details, as someone they may contact. Horizon Diet and Nutrition Advisors keeps a record of your name, contact details and dates and times of appointments on a password-protected database and in a locked filing cabinet for all paper-based documents. 

Our nutritionist is required to keep brief notes about the focus of your work. 

These are kept securely and are password protected. 

Any reports produced by Horizon Diet and Nutrition Advisors to evaluate the service contain anonymised data and will not contain any identifying information of clients.

Problems & Complaints

If you are having a problem with our nutritionist, or the service, which you have not been able to resolve with our nutritionist, you may raise it with our us who will support you in finding a resolution.

Working With Multiple Consultants at Horizon Diet and Nutrition Advisors

Our nutritionist may suggest you see other members of the Horizon Diet and Nutrition Advisors team support your goals. 

Information about your symptoms and condition will be discussed between team members in a secure area so that we can provide you with a coordinated treatment plan. 

Your consultant will ask for your informed consent to share information with the team and ask if there is anything that you want to remain confidential. 

Your therapist will help you assimilate any information gathered from other members of the team.

Working on Skype/Zoom, It is your responsibility of the client/patient to have a working internet connection for all Skype/Zoom sessions. You will be charged for scheduled sessions if your consultant is waiting for your call and you are unable to connect online.

Our Nutritional Therapists at Horizon Diet and Nutrition Advisors all practice following the British Nutrition Foundation & Nutritional Therapy’s ethical framework and in line with Horizon Diet and Nutrition Advisors policies regarding Equal Opportunities and Safety.

The Nutritional Therapy Descriptor Nutritional Therapy is the application of nutrition science in the promotion of health, peak performance and individual care. 

Nutritional therapy practitioners use a wide range of tools to assess and identify potential nutritional imbalances and understand how these may contribute to an individual's symptoms and health concerns.

This approach allows them to work with individuals to address nutritional balance and help support the body towards maintaining health.

Nutritional therapy is recognised as a complementary medicine and is relevant for individuals with chronic conditions, as well as those looking for support to enhance their health and wellbeing.

 Practitioners consider each individual to be unique and recommend personalised nutrition and lifestyle programmes rather than a 'one size fits all approach. 

Practitioners never recommend nutritional therapy as a replacement for medical advice and always refer any client with 'red flag' signs or symptoms to their medical professional. 

They will also frequently work alongside a medical professional and will communicate with other healthcare professionals involved in the client's care to explain any nutritional therapy programme that has been provided.

The Nutritional Therapist requests that the Client notes the following -

The degree of benefit obtainable from Nutritional Therapy may vary between clients with similar health problems and following a similar Nutritional Therapy programme.

Nutritional advice does not include meal recipes, our nutritionist only provides information in which foods are beneficial, which foods to avoid and which cooking methods should be avoided for better health.

Nutritional advice will be tailored to support health conditions and/or health concerns identified and agreed upon between both parties.

Nutritional therapists are not permitted to diagnose or claim to treat medical conditions.

Nutritional advice is not a substitute for professional medical advice and/or treatment.

Your Nutritional Therapist may recommend food supplements and/or functional testing as part of your Nutritional Therapy programme and may receive a commission on these products or services.

This document only covers the practice of Nutritional Therapy within this consultation, and your practitioner will make it clear if he or she intends to step outside this boundary.

The Client understands and agrees to the following

You are responsible for contacting your GP about any health concerns.

If you are receiving treatment from your GP or any other medical provider you should tell him/her about any nutritional strategy provided by a Nutritional Therapist. 

This is necessary because of any possible reaction between medication and the nutritional program.

You must tell your Nutritional Therapist about any medical diagnosis, medication, herbal medicine or food supplements you are taking as this may affect the nutritional program.

If you are unclear about the agreed program/food supplement doses time, you should contact your Nutritional Therapist promptly for clarification.

You must contact your Nutritional Therapist should you wish to continue any specified supplement program for longer than 3 months, to avoid any potential adverse reactions. In any case, we recommend a regular review of supplements to ensure they remain appropriate for your needs.

You are advised to promptly report any concerns about your program to your Nutritional Therapist for discussion/action.

Please note we do recommend that all supplements are taken at different times of the day to any prescribed medications.

You permit us to contact GP regarding any agreed aspects of my case.

I accept it is important that I tell my nutritional therapist about any medical diagnosis, medication, herbal medicine, or food supplements, I am taking as this may affect the nutritional programme.

If I am unclear about the agreed nutritional therapy programme/food supplement doses/time period, I should contact my nutritional therapist promptly for clarification.

I must contact my nutritional therapist should I wish to continue any specified supplement programme for longer than the originally agreed period, to avoid any potential adverse reactions.

I give consent to Horizon Diet and Nutrition Advisors Ltd to record my consultations via any form of electronic media or paper-based with permission of both me and my Nutritional Therapist.

We would always recommend you discuss any dietary or supplemental concerns or changes you wish to make with your G.P. Medication should never be discontinued or dosage amended without your G.P.’s prior knowledge and agreement.

The information on any document is published in good faith and for general information purpose only.

Our recipes does not make any warranties about the completeness, reliability and accuracy of this information. 

Any action you take upon the information you find on this document, is strictly at your own risk. 

Our food recipes will not be liable for any losses and/or damages in connection with the use of our services.

By using our website, you hereby consent to our disclaimer and agree to its terms.

Client/Patient consent details:

All the information given is accurate and true to the best of my knowledge. I understand that Nutritional therapy is a complementary therapy and is not intended to replace conventional medical intervention. I am aware that the advice of Horizon Diet and Nutrition Advisors Ltd Nutritionists are not intended to replace or negate the advice of qualified medical personnel.

I am aware of the associated risks of Nutritional therapy and I hereby waive Horizon Diet and Nutrition Advisors Ltd, Its Managing Director, its employees and associates from any liability of ill-health, injury, loss or damage to personal property that I may incur as the result of Nutritional Therapy.

By the customer/individual/client/patient ticking the consent box on our website shop or questionnaire form and submitting the questionnaire form to Horizon Diet and Nutrition Advisors Ltd, has therefore accepted and agreed to all the consent agreements and all terms and conditions.
