Terms and Conditions of Service


1. About Us


Horizon Diet & Nutrition Advisors Ltd (“we”, “us”, “our”) provides online nutritional therapy and wellbeing consultations to clients aged 18 years and over.


All services are delivered remotely via Goggle meeting. We do not provide in-person consultations.


2. Acceptance of Terms


By completing our client questionnaire and ticking the acceptance box, you confirm that:


You have read, understood and agree to these Terms and Conditions


You consent to receiving nutritional therapy services


You agree to our Privacy Policy and Data Protection practices


You confirm that the information you provide is accurate and complete


You acknowledge that all advice is based solely on the information you provide


If you do not agree with these Terms and Conditions, you must not use our services.


3. Eligibility for Services


Our services are available only to:


Individuals aged 18 years and over


Clients who are legally permitted to receive nutritional therapy services


Clients who are not United States citizens or residents, due to insurance policy limitations


We reserve the right to refuse or discontinue services if eligibility requirements are not met.


4. Nature of Nutritional Therapy Services


Our services include nutritional therapy for:


Weight Loss nutritional therapy 


IBS nutritional therapy


Perimenopause nutritional therapy


PCOS nutritional therapy


Type 2 Diabetes nutritional therapy 


Health and wellbeing


High Blood Pressure


Lifestyle and dietary guidance


Nutritional therapy is not a substitute for medical diagnosis, treatment or medication.


We do not:


Diagnose medical conditions


Prescribe medication


Provide emergency medical services


Clients must seek advice from a GP or medical professional for any medical concerns.


5. Client Responsibilities


You confirm that:


All information provided on your questionnaire is truthful and accurate


You will disclose relevant medical history, medications and health conditions


You understand advice is based solely on the information you provide


You take full responsibility for implementing any recommendations


Horizon Diet & Nutrition Advisors Ltd will not be liable for consequences arising from incomplete, misleading or inaccurate client information.

7. Online Consultations (Goggle meeting Only)


All consultations are conducted exclusively via Goggle meeting.


Clients are responsible for:


Having a stable internet connection


Google Meet Audio only can be used to improve internet connect quality. 


Ensuring a private and suitable environment


Logging in on time for their scheduled appointment


Missed or delayed connections may be treated as late cancellation.


8. Booking and Payments


All bookings and payments are processed via Setmore.


Accepted payment methods include:

Stripe


PayPal


Visa


Mastercard


American Express (Amex)


Discover


JCB


Diners Club


Maestro


Apple Pay


Google Pay


Full payment is required at the time of booking.


9. Cancellation and Refund Policy


24-Hour Cancellation Policy


Clients must give at least 24 hours’ notice to cancel or reschedule an appointment.


Cancellations made more than 24 hours before the appointment will be refunded or rescheduled


Cancellations made within 24 hours of the appointment are non-refundable


Failure to attend without notice will be charged in full


Refunds will be processed via the original payment method where applicable.


Refunds will be offered if our nutritionist is not able to successfully provide nutritional therapy due the following reasons: 


Patient/client has severe health conditions and is needing of a Dietitian.


Any of our nutritionist has any interest access issue and isn't able to attend session.


If our nutritionist cancels session before appointment time and date.

 

10. Confidentiality


We are committed to maintaining strict client confidentiality.


All client information is treated as confidential except where:


Disclosure is required by law


There is a serious risk of harm to the client or others


The client provides written consent


11. Data Protection and GDPR


We comply with the UK Data Protection Act 2018 and the UK General Data Protection Regulation (UK GDPR).


We collect and store:


Personal data


Health information


Consultation notes


This data is used solely for providing nutritional therapy services.


Clients have the right to:


Access their data


Request corrections


Request deletion (where legally permissible)


Withdraw consent at any time


Data is securely stored and never sold to third parties.


12. Staff Protection Policy


We operate a zero-tolerance policy towards:


Harassment


Abuse


Threatening behaviour


Discrimination


Any inappropriate behaviour may result in immediate termination of services without refund.


13. Limitation of Liability


Horizon Diet & Nutrition Advisors Ltd shall not be liable for:


The practitioner will use reasonable care and skill in providing nutrition therapy services. 


However, individual results may vary and no specific outcomes or results are guaranteed.


To the fullest extent permitted by law, the practitioner shall not be liable for any direct, indirect, incidental, or consequential loss, injury, or damages arising from the use or misuse of advice, recommendations, supplements, or nutritional guidance provided.


Clients acknowledge that they participate in nutrition therapy voluntarily and accept full responsibility for their own health decisions and implementation of any recommendations provided.


14. Service Availability


We reserve the right to:


Modify or discontinue services


Refuse service to any client


Update these Terms and Conditions


Any updates will be published on our website.


15. Governing Law


These Terms and Conditions are governed by and interpreted in accordance with the laws of England and Wales.


Any disputes shall be subject to the exclusive jurisdiction of the courts of England and Wales.


16. Gender Recognition


For the purposes of providing nutritional therapy services, gender is recorded based on biological sex at birth, as this may be relevant to nutritional assessment and health considerations. 


All clients will be treated with dignity and respect at all times.


17. Supplement Use and Client Responsibility


Any nutritional supplements recommended as part of a consultation or programme are suggested based on the information provided by the client at the time of consultation.


The client acknowledges that they are responsible for:


Following the dosage and usage instructions provided by the practitioner.


Reading and following the instructions and safety information supplied by the supplement manufacturer.


Informing the practitioner of any existing medical conditions, medications, allergies, or changes to their health status before beginning any supplement protocol.


The practitioner cannot be held responsible for any adverse effects, reactions, or outcomes resulting from:


Failure to follow the recommended dosage or instructions.


Use of supplements not specifically recommended by the practitioner.


Combining supplements with medications or other products without notifying the practitioner.


Misuse, overuse, or alteration of the recommended supplement protocol.


Clients are advised to consult their GP or relevant healthcare professional before beginning any new supplement regimen, particularly if they are pregnant, breastfeeding, taking medication, or managing a medical condition.


18. Individual Results Disclaimer


Nutrition therapy outcomes vary between individuals and depend on multiple factors including lifestyle, medical history, genetics, and adherence to recommendations.


While the practitioner will provide evidence-informed guidance and personalised recommendations, no guarantee of specific results or outcomes can be made.


19. Disclosure of Health Information


Clients are responsible for providing accurate, complete, and up-to-date information regarding their health history, medical conditions, medications, allergies, and supplements.


The practitioner relies on this information to provide appropriate nutritional guidance. The practitioner shall not be held liable for any consequences arising from inaccurate, incomplete, or withheld health information.


20. Medical Disclaimer


Nutrition therapy and lifestyle advice provided by the practitioner are intended to support general health and wellbeing and are not a substitute for medical advice, diagnosis, or treatment.


The practitioner does not provide medical services and does not diagnose medical conditions. Clients are advised to consult their GP or qualified healthcare professional regarding any medical concerns, symptoms, or conditions.


Any recommendations provided are based on the information disclosed by the client during consultations. The practitioner cannot be held responsible for outcomes resulting from incomplete, inaccurate, or withheld health information.


21. Consent to Treatment


By ticking the Terms and Conditions box on the questionnaire form, you give informed consent that:


You understand the nature of nutritional therapy


You understand that results cannot be guaranteed


You accept that changes to diet and lifestyle are undertaken at your own risk


You agree to follow professional medical advice alongside nutritional guidance
